New & Transfer Patients--You can schedule your first appointment in the portal!
**You must have your insurance information on file for our office to verify your benefits. If we cannot verify benefits, we expect payment at the time of service. See the Good Faith Estimate in Quetisonnaires for our fee schedule, which is based on billing, but discounted for cash pay patients.
**You should verify Jennifer Paulson FNP (NPI 1164685210, EIN 472245016) is in your insurance network. We verify and bill your insurance as a courtesy, sometimes the information is not available to us. We are not responsible for what insurance does not cover.
**If you sign up for monthly membership as a new patient, we will contact you and offer you an appointment time within 3 weeks of registering!
**Transfer Patients: Please complete the Transfer Patient forms in the Questionnaires section. If you saw Dr Paulson at either FMI or Elixir, please fill out the Authorization to Release Medical Records accordingly as soon as possible so that we may obtain your records promptly.
**New Patients: You will be contacted via the portal by Laura with further instructions on required forms. We ask that you complete these at least 1 week prior to your scheduled appointment. If you fail to complete these forms, your appointment time will be used to go through them and this will NOT be a typical functional medicine initial visit.
Membership & Portal Messaging:
Please know our portal messaging is secure. Laura manages all portal messages and will route messages to Dr. Paulson for review if you are a member. If you are not a member, you will likely hear back from Laura first to confirm you want to incur a portal message fee to have your concern addressed by Dr Paulson (if it cannot wait until next appointment). Please consider membership if you want direct access to portal messages with Dr. Paulson between visits.

Blue Cross/Regence, Regence (limited to telehealth visits only due to very low reimbursement for in person visits and no state individual plans purchased thru the marketplace), Pacific Source, Select Health (Bright Path & St Lukes plans), St. Lukes Commerical plans. Cigna/Providence
UHC commercial plans (no marketplace individual plans)
*We are not contracted with Medicare or Medicaid, Aetna, MODA-we do offer a discount for our cash pay patients. You can find our fees and discounted pricing in the portal under Good Faith Estimate.
